Sometimes for me it has to do with the fact that I don't like to see decayed or mutilated faces; I can deal with other body parts, but faces just stick with me, which is one of the factors behind my decision not to pursue physical anthropology as a career. (Before you met me, there was a point at which I was considering it.)
I'm not generally a pussy about blood and guts, but if I'm going to enjoy the movie, then I need the blood and guts to have a purpose and not just be there for their own sake, which is my issue with a lot of horror flicks. I watch movies for a lot of reasons, but "being grossed out" isn't one of them.
